Just a quick response:

Delcam has a module to their CAD package called Artcam that does just that
turns greyscale values of images into a 3dsurface of varying heights.

Alias also has the same functionality but has slightly more control of the
surface.

We looked at this technique to apply modified textured surfaces to mould
cavities
