Look in the Photonics Spectra Buyer's Guide under lenses, custom. (Google
to find web version.)
 
That's a difficult shape to do as a one off or prototype, but you can
likely get the same result using a flat window of the right diameter and one or
two semi-convex lenses optically cemented together. You can probably get
everything you need from Edmund Scientific for a few bucks doing it that
way.
